Surgical Consent Forms Repository

Informed consent is the ethical and clinical cornerstone of safe, respectful surgical care. Review our guidelines and download the relevant procedure consent forms.

The Principle of Informed Consent

Before undergoing any surgical procedure, you must fully understand the nature of the operation, its anticipated benefits, the potential alternatives, and the associated surgical and anaesthetic risks.

During your consulting room session, Dr Aliashkevich will comprehensively outline your surgical plan and answer all questions. We provide these consent documents early so you can review them in comfort with your family and GP.

Consent for Spine Surgery

Covers microdiscectomy, cervical/lumbar disc replacement, decompression (laminectomy), and spinal fusion.

Submitting Your Consent Form

Signed consent forms must be returned to our office at least 48 hours prior to your scheduled hospital admission date. You can return them by emailing a scanned copy to admin@mineuro.com.au or bringing the physical, signed copy to your final pre-operative consultation.
